This Short Note provides an overview of chronic disease management in European health systems, emphasising the diversity of existing models and their key elements.

The review of different approaches to chronic disease management emphasises the absence of a single model. This diversity reflects the complex nature of chronic conditions and the responses required to meet the varied needs of patients and the capacities of health systems. There is a growing focus on the role of patient self-management and the digital transformation of health, which reflect the changing paradigms in healthcare. International experience highlights the importance of the context of each health system in defining the best chronic disease management model. A central element of success is the coherence between the organisational and financial elements used in the chronic disease management model.
